Overview
We are seeking an experienced and compassionate Home Health Admission Registered Nurse (RN) to join our home health care team in the Greensburg and Irwin, Pennsylvania areas. Key Responsibilities
Perform comprehensive Start of Care (SOC) assessments for new home health patients
Develop and manage individualized care plans tailored to each patient's needs
Deliver direct nursing care in accordance with physician orders and care standards
Collaborate with interdisciplinary team members to ensure high-quality outcomes
Maintain accurate and timely documentation using EMR systems and mobile technology
Manage daily schedule and prioritize patient visits efficiently
Educate patients and families about care plans, medications, and follow-up needs
Schedule and Shift Details
Schedule: Monday Friday, 8-hour shifts
On-call: One evening per week (5:00 p.m. 10:00 p.m.) and one weekend per month (Saturday and Sunday)
Qualifications
Required
Associate degree in Nursing or higher
Current Registered Nurse (RN) license in the state of Pennsylvania
Valid Driver's License and reliable transportation
Minimum of 1 year of experience with electronic medical record (EMR) systems
Current Basic Life Support (BLS) certification from the American Heart Association or equivalent
Preferred
Advanced Cardiac Life Support (ACLS) certification
Experience in home health admissions, community nursing, or skilled home care
